I have a feeling that the fact that NPC dialogue doesn't change on ascending through factions is a time/money concession rather than a oversight, given that they have to record each and every line with an expensive voice actor. Having said that, it does grate somewhat being referred to as "brynjolf's new protegee" when I'm... the guild master.
I agree though that being elite in the associated skills would make becoming a faction leader much more rewarding though, as well as some consistency- if you rock up to the thieves guild in dark brotherhood armour it'd be nice if they responded! I think that decision might have been to stop the game from becoming too restrictive for casual players who only want to have one character.
As for making there be more options in the questline, and no unkillable NPCs, if you think about it, in order to cover every option it would take a ridiculous amount of dialogue scripting and recording in an already detailed game. Also it would be infuriating for a casual player to kill someone by accident and have a whole lot of plot cut off from you!
And fast traveling- you don't have to use it!
My personal gripe is some lazy design resulting in intersecting apparel- only bows and quivers of the same type don't intersect weirdly and tails an capes are quite frankly silly. Regulate quiver sizes! Make capes solid!
