The game explanation is that it's too much work for a feature that would probably be underappreciated anyway...
The lore explanation is that Skyrim is just really sparsely populated, and everybody is just really spread out. Not that many people live in the big cities. In TES lore, there are a LOT more villages and cities in Skyrim too, but because of time, space, and platform constraints, they couldn't be added. I'd expect more territory in TES 6 however.
