It's a Forsworn headdress, head over to the Reach and clear out a Forsworn camp and you'll be certain to get one.

Yea, it's just the foresworn headdress. Or if you you do the Markarth murder mystery thing you can get it as a gift called "helm of the old gods" but once you get your enchant up you can probably make a better version using normal foresworn headdress on your own.. Although the female version if you play a female character will not look like this; it just looks like some tied up dreds.

@ Static & Xaraan. Thankyou guys - I haven't come accross the Forsworn yet, but Delphine has just warned me that there might be lots where I'm going next, so hopefully I'll pick one up there. Little bit disapointed that the female version doesn't include antlers though. My Khajiit is a hunter/assasin type character, and it would look awesome.
